摘  要:目的 分析肥胖哮喘患者的激素抵抗机制及其细胞因子特征。方法 选取2017年1月-2017年12月我院收治的肥胖哮喘患者50例,将其作为研究组,选取同时期来我院检查的50例健康者,将其作为对照组,观察两组患者体内激素抵抗机制以及细胞因子特征的差异。结果 细胞因子方面,研究组患者白细胞介素6(IL-6)、白细胞介素10(IL-10)以及肿瘤坏死因子-α(TNF-α)明显高于对照组正常水平(P<0.05);痰细胞分类方面,研究组患者的中性粒细胞以及巨噬细胞所占的百分比明显低于对照组正常水平,并且嗜酸性粒细胞所占的百分比明显高于对照组正常水平(P<0.05);激素抵抗机制方面,激素抵抗性患者激素受体β单位表达、活化蛋白I(AP-I)表达增高以及热休克蛋白90异常的发生率明显高于对照组患者(P<0.05);血清中因子水平,研究组白细胞介素12(IL-12)明显低于对照组,而白细胞介素4(IL-4)以及免疫球蛋白(IgE)明显高于对照组(P<0.05);各种细胞因子上升,激素抵抗上升,二者存在显著相关性(P<0.05)。结论 肥胖患者哮喘发生的过程有大量细胞因子的参与,同时参与激素效应降低的过程,炎症因子的水平也可以作为患者临床疗效的判断指标,激素抵抗性的发生会增加患者和社会负担,临床上应进一步的研究。Objective To analyze the steroid-resistant mechanism and its cytokine characteristics in obesity patients with asthma.Methods 50 obesity patients with asthma treated from January 2017 to December 2017 in our hospital were selected as the study group,and 50 healthy check-ups were selected as the control group.The steroid-resistant mechanism and its cytokine characteristics were analyzed.Results The interleukin-6(IL-6),interleukin-10(IL-10)and tumor necrosis factor-α(TNF-α)in the study group were significantly higher than those in the control group(P<0.05).The ratio of neutrophile granulocyte and macrophage was lower,and the ratio of eosinophilic granulocyte was higher in the study group than in the control group(P<0.05).The rate of increased hormone receptor-βexpression and activated protein I(AP-I)expression and abnormal heat shock protein 90 in the steroid-resistant asthma patients was significantly higher in the study group than in the control group(P<0.05).The level of IL-12 in the study group decreased,and the level of IL-4 and IgE increased,and it had significant difference between the two groups(P<0.05).The increased cytokines and hormonal resistance were mutually correlated with each other(P<0.05).Conclusion As asthma is occurred,many cytokines are involved in reducing hormone effect.The inflammatory factors are considered as the diagnosis indicators,and the increasing steroid resistance will bring about the patient s and society's burden.It is to be explored in future.
